APPENDICES & SUPPORTING FORMS
The following appendices, forms, declarations, and supporting documents may be used by KPRF for administration, membership, race management, discipline, finance, and record keeping.
These appendices shall form part of the official administrative records of KPRF and may be updated, modified, replaced, or newly introduced by the KPRF Leadership / Managing Committee whenever required.
Appendix A: Membership Application Form
To be used by new applicants seeking KPRF membership.
Appendix B: Member Undertaking & Rules Acceptance Form
To confirm that the member has read, understood, accepted, and agreed to follow the KPRF Constitution, Bylaws, race rules, discipline rules, and official decisions.
Appendix C: Minor Participation / Guardian Undertaking Form
To be used when a minor participates under the responsibility of a registered adult guardian.
Appendix D: Race Entry Form
To record member name, loft name, pigeon ring details, race category, entry fees, and declaration of ownership.
Appendix E: Loft Registration & GPS Declaration Form
To record official loft address, GPS coordinates, contact details, and distance verification details.
Appendix F: Ring Registration Form
To record ring number, ring year, ring type, pigeon details, ownership, and eligibility for KPRF races.
Appendix G: Race Protest / Objection Form
To submit official objections regarding race results, timing, distance, ring details, eligibility, basketing, liberation, or any race-related issue.
Appendix H: Disciplinary Complaint Form
To report fraud, misconduct, doping, ring tampering, abuse, defamation, financial irregularity, or any violation of KPRF rules.
Appendix I: Sponsorship / Donation Declaration Form
To record sponsorship amount, donor details, purpose, race/event support, and approval by KPRF.
Appendix J: Equipment Issue & Return Form
To record issue and return of race baskets, crates, clocks, ETS systems, seals, documents, or any KPRF property.
Appendix K: Prize Receipt & Acknowledgement Form
To record receipt of cups, trophies, certificates, medals, cash prizes, special awards, or sponsored prizes.
Appendix L: Resignation Form
To be used by members who wish to resign from KPRF officially.
Appendix M: Re-Admission Application Form
To be used by resigned, inactive, removed, or expelled members seeking re-admission, subject to KPRF approval.
Appendix N: Amendment Register
To record all future amendments, corrections, additions, deletions, and official clarifications made to the Constitution and Bylaws.
Appendix O: Official Signature & Seal Page
To contain the signatures of the Permanent President, Founder, Secretary, Treasurer, Managing Committee Members, and official KPRF seal where applicable.

Final Note:
All appendices and supporting forms shall be used for administrative convenience and official record keeping.
If any appendix conflicts with the main Constitution and Bylaws, the main Constitution and Bylaws shall prevail.
The KPRF Leadership / Managing Committee shall have final authority to create, approve, modify, or discontinue any appendix or form according to the needs of the Federation.
